The office is easily accessible from I-235. Take 22nd St/West Des Moines exit, Exit 2. Turn North onto 22nd Street. After the second stoplight take the first driveway on your right. Clinic is located on the upper level, back of the building. Ste #201.

Emergencies:

If you have an emergency or are severely ill after hours please contact the on call physicians, call 911, or go to the nearest urgent care facility or hospital.

Appointments:

Patients are seen by appointment and for walk in medical needs. Please arrive 10 minutes early to fill out necessary paperwork.

Insurance and Payments:

Barazanji Family Medical Clinic contracts with most insurance companies including Medicare and Medicaid. We accept cash, checks, Visa and MasterCard. If you have questions regarding a specific insurance plan, please feel free to contact our office. All co-pays are due at time of service. Please bring your insurance card & photo ID to all office visits. Notify the office of changes in your address or phone number.

Medication Refills:

Medication refills will be offered at scheduled office visits. If you have refills on your prescription please phone your pharmacy to obtain that refill. When requesting a prescription refill contact your pharmacy who will fax us the request. Please allow 24 – 48 hours for the clinic to process the request.

After Hours Phone Calls:

An on call physician is available after hours and on the weekends to help patients decide the proper course of treatment needed. This service does not take place of an office visit.

Completion of Medical Forms:

Due to the increasing demand for and complexity of disability, return to work, and FMLA forms, please allow 5 -10 business days for completion of the forms. An office visit must precede the completion of these forms.
